At the start of the game, you get to choose from 4 instantly generated island layouts. If you don’t like any of them, you can just restart the game and get 4 new choices. No matter which Island you choose, the size will be the same.
You cannot have more than one Island
No, everyone in the game gets only 1 base island where they can build their home, including resident services and airport.
But you can travel to other Islands
Yes, although you will have to go to someone in your friend’s list or get a Nook Miles ticket to go to a random one.
Do not go through the resetting process every time something doesn’t suit you. When you get the Island modifier you will be able to change the terrain, rivers, cliffs and make it to your liking.

Even though you can reset Island Layout, Name and Hemisphere remains the same.

If the Resident Services are on the centre of the island get your hands on it immediately. Since you will be using it quite frequently.
Other variables are very undermined, you can reset every time if you want any specific fruit or villager. Select an island that is spacious around resident services. Fewer ponds are recommended. Symmetrical and river doesn’t take up too much space.
