When you fill the form, PES would be cross-checking if you are a valid user (this comes in the case of players not utilising a Konami ID). Henceforth, fill the form and then select the Accept button. From here, PES 2021 Mobile will redirect you to a page. You will get to have a form on this page asking to verify the lost account credentials you want to recover. This form is to verify and cross-check your claim. You need to link your Google Play account with the new/current Owner ID. Konami would be using it to recover the data of the Owner ID you are trying to recover.
For this, you need to share credentials. This includes the name of the owner, the email address. You also need to use the current ID to restore data of the Account ID you reported for recovery. Henceforth, Konami would also be asking for additional information, like Team Name, myClub coins. Using this information, Konami would start processing in final verification and start recovering your previous account.
